Habilitation Care is designed for adults (18+) with developmental or intellectual disabilities seeking structured, goal-oriented skill development.
Our Services
Structured, person-centered programs designed to build life skills, develop independence, and strengthen community participation.
Personalized training in self-care, meal preparation, household management, money skills, and health literacy — building real independence one step at a time.
Structured activities that strengthen cognitive abilities, motor skills, communication, social interaction, and creative expression through engaging, enjoyable methods.
Compassionate, strengths-based coaching to build resilience, manage anxiety, develop coping strategies, and encourage self-advocacy and empowerment.
Guided outings, volunteering, employment support, and social activities that strengthen inclusion, build relationships, and foster genuine belonging.
Transparent, frequent communication with families; comprehensive progress documentation; and collaborative goal-setting to celebrate growth together.
HorizonCare offers an integrated continuum — Companion Care, Habilitation Care, and Personal Care. As needs evolve, clients transition seamlessly between services with the same trusted team.

Getting Started

Our habilitation coordinator meets your loved one in-home to understand their strengths, interests, goals, and support needs across all life domains.
We match your loved one with a habilitation specialist based on personality, interests, communication style, and cultural background.
Introductory sessions to build rapport, develop the individualized habilitation plan, and confirm the match. Not right? We re-match at no extra cost.
Regular skill-building sessions, quarterly progress reviews, family updates, and plan adjustments as your loved one grows.
Our Specialists
Every habilitation specialist completes 60–80 hours of initial training and 30+ hours of annual development. We recruit for empathy, teaching ability, and genuine belief in every client's potential.
