Person-Centered Care in Ontario
HorizonCare Wellness provides companion care for older adults and habilitation support for adults with developmental disabilities — unified by one philosophy: we work with people, never just for them.

Who We Are
HorizonCare Wellness is a Toronto-based health services organization offering integrated, person-centered support across two specialized divisions. Whether your family needs companionship for an aging parent or habilitation support for an adult child with a developmental disability, we bring the same values, the same training standards, and the same commitment to dignity.
“We don't provide care to people — we provide care with people.”
The Continuum
Needs change. Our continuum means clients never have to start over with a stranger. The same trusted team adjusts the level of support as life evolves.
Non-medical home support for older adults — companionship, wellness check-ins, meal preparation, transportation, and care coordination that reduces isolation and restores joy.
Structured, goal-oriented support for adults with developmental and intellectual disabilities — building life skills, strengthening community connections, and celebrating every step toward independence.
Hands-on daily living assistance for individuals who need a higher level of physical support — bathing, grooming, mobility, and medication reminders delivered with the same person-centered philosophy.
Our Values
Across every division and every client relationship, these shared principles guide how we hire, train, and deliver care.
Every care plan is built around the individual — their story, their goals, their rhythm.
We protect autonomy and respect choices. Independence is celebrated, never replaced.
Language, traditions, and cultural identity are honoured in every client match and care plan.
Families are experts and partners. Transparent communication and collaborative goal-setting are standard.
Familiar faces build trust. Clients keep their care team as needs evolve across the continuum.
Training, methods, and reviews grounded in current research and best practices.
